Calhoun County launches form for essential worker organizations

CALHOUN COUNTY, Mich. — A vaccination request form is now available on the Calhoun County website for essential frontline organizations that meet Phase 1B criteria, the county’s health department said Friday.

That includes seniors over 65, teachers, corrections officers, homeless shelter staff, critical infrastructure staff, food manufacturing, veterinarians, court staff, public transit workers, grocery store workers and more.

The Calhoun County Public Health Department is asking for a point of contact of the organization to fill out the form on behalf of the organization they represent.

This form isn’t intended for individual frontline workers’ submissions.

Once the department receives a request, it will work with the organization to schedule a clinic.

Because COVID-19 vaccines are limited in Michigan, appointments may not yet be available.
